EMPLOYEE ASSISTANCE PROGRAM (EAP)

The EAP provides free counseling services for you and your dependents. It is completely confidential and paid for by The Ōnin Group. Counselors are available 24/7/365. The EAP offers many ways to assist you with life challenges, such as: Relationships, Health, Financial, Legal, Personal Growth, and Emotional Wellbeing. There is no copay or out-of-pocket cost for EAP services!

    The following vision plan is provided at NO COST to all Teammates when enrolled in a medical plan:

    Eye Exam Covered 100% after $10 copay
    Contacts (in lieu of eyeglass lenses) Up to $100 allowance
    Lenses: Single, Bifocal or Trifocal Covered 100% after $25 copay
    Standard Frame Up to $100 allowance, up to 20% off amount over allowance

    DENTAL PLAN

    The Ōnin Group will continue to offer Teammates dental coverage! The following is a summary of the coverage available to you:

    Common Questions

    Once you have received your second paycheck from Ōnin Staffing, it’s time to sign up for your benefits! Visit worxenroll.com/oningroup or call the Enrollment Assistance line (1-833-236-7463) to sign up for the insurance plan you would like. You must do this before you have worked with Ōnin a total of 30 days after your start date.  

    If you do not call the Enrollment Assistance line (1-833-236-7463) or visit  worxenroll.com/oningroup within this time period, you will be automatically signed up for the Teammate/MEC Medical Plan for individual coverage 30 days after your start date. When you are automatically signed up for insurance, this is called “automatic enrollment.”

    Your login information is as follows:

    Basically, both terms describe the times when you sign up for health insurance.

    Open Enrollment: a specific period in November through December when Americans can enroll in a health insurance plan. You can sign up for an insurance plan through the government healthcare plan or choose a plan through your employer if they offer insurance benefits. The Ōnin Group companies do offer insurance, so if you are employed by an Ōnin Group company and it’s during the November-December time period, it’s time to choose your plan! Ōnin’s 2018 Open Enrollment period is November 19th through December 12th. During this time, everyone who is an Ōnin employee has the chance to sign up for insurance through December 12th.

    Special Enrollment Period: this period occurs after a special life event that allows you to sign up for health insurance outside of the normal Open Enrollment November-December time period. Special events include getting hired at a new job, getting married or having a baby. What does this mean for people who work for Ōnin Group companies? When you are newly hired, you need to sign up or opt-out of insurance after you’ve received your second paycheck and before you have reached 30 days from your start date, which is your Special Enrollment Period.

    Whether you are signing up during the Open Enrollment period or you have just been hired with Ōnin and need to sign up for insurance during the Special Enrollment Period, it’s important to select the benefits option that works best for you and your family or opt-out of a plan if you do not want insurance. If you do not choose a plan or opt-out, you will be automatically enrolled in the Teammate/MEC individual medical plan.

    You will be able to use your Ōnin insurance plan the pay period following 30 days of employment with Ōnin. You will receive an insurance card in five to seven business days after your first 30 days have passed. If you do not receive this card, call the Advocate line (1-833-751-1096) for help.

    30 days after you begin working for Ōnin Staffing, you will have benefits. The only reason you will not have benefits is if you specifically opt-out of insurance within the 30-day period by calling the Enrollment Assistance line (1-833-236-7463) or visiting worxenroll.com/oningroup.com.

    If you have downloaded the Teammates App, you will see a reminder notification telling you when you are about to be enrolled. You will also be sent a reminder postcard to the address you gave your local Ōnin office.

    You must work with Ōnin for 30 days to receive health insurance. You will be automatically enrolled in the Teammate/MEC Medical Plan after you have worked with Ōnin for 30 days unless you took action and declined your benefits or selected a different plan. After you receive your second paycheck, you can choose the benefits option that works best for you or opt-out of coverage.

    When you are signed up for the Teammate/MEC Medical Plan, you get the following:

         Teammate/MEC Medical Plan: Look at the chart called “Teammate/MEC Medical Plan” below for the details.

         Teledoctor Service: Talk to a doctor and they will prescribe you the medication you need for FREE from your phone.

         Vision Plan: Look at the chart called “Vision Plan” for the details.

         Employee Assistance Counseling Service: Talk to a mental health counselor at any time, as often as you would like for FREE. Use this service if you are having problems like depression or anxiety.

    If you signed up for or were automatically enrolled in the individual Teammate/MEC Medical Plan, only you can use these benefits. If you signed up for a plan that includes your children and/or your husband or wife, they can use all of these benefits as well.

    Yes, you can cover your husband or wife (also known as your “spouse”) and your children (also known as “dependents”) with the Teammate/MEC Medical Plan, Bronze Plan and Dental Plan. To cover your spouse and dependents, visit worxenroll.com/oningroup or call the Enrollment Assistance line (1-833-236-7463).

    Call the Enrollment Assistance line (1-833-236-7463) or visit worxenroll.com/oningroup and select a plan that covers your family’s needs. If you have any questions, call the Advocate line (1-833-751-1096).

    If you do not want the Teammate/MEC Medical Plan, you need to call the Enrollment Assistance line (1-833-236-7463) and say you do not want to be enrolled in the Teammate/MEC Medical Plan. You must do this before you have worked with Ōnin Staffing for 30 days. When you tell us you do not want the Teammate/MEC Medical Plan to be your insurance, this is called “opting out.”

    As soon as you’ve received your second check from Ōnin, you can opt-out.  You must call the Enrollment Assistance line (1-833-236-7463) before you have worked with Ōnin for 30 days.

    You have two health insurance options:

         Teammate/MEC Medical Plan: This plan makes healthcare both affordable and easy to use. You will be automatically signed up for this plan if you do not select a different plan or opt-out within your first 30 days of working with Ōnin.

         Bronze Plan: This plan is more expensive than the Teammate/MEC Medical Plan, but it may be the best option for some people. Discuss your options by calling the Advocate line (1-833-751-1096).

    If you have received your second paycheck from Ōnin, you can sign up for the Bronze Plan. To sign up for the Bronze Plan, you must call the Advocate Line (1-833-751-1096) or visit worxenroll.com/oningroup to select this plan before you have worked with Ōnin for 30 days.

    You can sign up for the Dental Plan after you receive your second paycheck from Ōnin.

    To sign up for the Dental Plan, call the Enrollment Assistance line (1-833-236-7463). You must call this number before you have worked at Ōnin Staffing for 30 days to get Dental Plan insurance. When you are automatically enrolled in the Teammate/MEC Medical Plan, you will not be automatically signed up for the Dental Plan. If do not sign up for an Ōnin insurance plan (the Teammate/MEC Plan or Bronze Plan), you cannot sign up for the Dental Plan.

    Once you have been enrolled in any of our benefits plans, you can keep that plan until December 31, 2019, or until you stop working with Ōnin Staffing. If you still work for Ōnin in November of 2019, you will choose your new plan for 2020.

    If you were signed up for an individual plan, you will be automatically enrolled in an individual plan this year.

    If you were automatically signed up for an individual + spouse plan last year, you will automatically be signed up for an individual + spouse plan this year.

    If you had the 2018 MEC Plan last year, you will be automatically enrolled in the Teammate/MEC Medical Plan.

    If you had the 2018 Bronze Plan, you will be automatically enrolled in the 2019 Bronze Plan.

    If you were enrolled in dental insurance last year, you will automatically be enrolled in dental insurance this year.

    If you were not enrolled in dental insurance last year, you will not be enrolled in dental insurance this year.

    All other Teammates will be enrolled in the Teammate/MEC Medical Plan for individuals this year.

    Yes. If you opted out of Ōnin insurance last year, you must still call the Enrollment Assistance line (1-833-236-7463) to opt-out again this year. If you do not opt-out this year, you will automatically be enrolled in the Teammate/MEC Medical Plan in 2019 even if you did not have insurance through Ōnin last year.

    Your premium (which is the amount of money taken out of your paycheck each week) is:

         Teammate (Individual): $17.82; everyone who is automatically enrolled will have this amount deducted from their paycheck. This option only covers you.

         Teammate & Spouse: $46.38

         Teammate & Childr(en): $56.46

         Family Plan: $80.69

    You will start paying your premium (the amount of money taken out of your paycheck each week) the first pay period after you have reached your 30 days of working with Ōnin. If you are taking part in the November 2018 Open Enrollment, you will start paying the first week of January. As soon as this happens, you can begin using your insurance plan. Remember, you should receive an insurance card that you can use around this time.

    You will receive an insurance card in the mail five to seven days after you have worked with Ōnin for 30 days. To use your insurance, first, find a doctor who accepts your plan by calling the Advocate line (1-833-751-1096) or visiting this link and then selecting: “PHCS Plan” and “Limited Benefit Plan.” Once you have found a healthcare provider, give the healthcare provider your insurance card information, and they will help you with the rest of the process.

    There are three options you can use to find a doctor:

    You have three options to talk with a doctor any time, as many times as you would like for free:

    No. You cannot cancel your Teammate/MEC Medical Plan, Bronze Plan or Dental Plan once you have been enrolled unless you have a federally qualifying life event. Once you have the insurance plan, you have to keep the plan until the end of the year.

    The only way you can get rid of your Teammate/MEC Medical Plan, Bronze Plan or Dental Plan before January of 2020 is if you have either of these situations happen:

         You stop working for Ōnin: In order for you to lose your benefits, you must stop working with Ōnin for at least 13 weeks. If you are hired full-time with another company and stop working with Ōnin, quit Ōnin Staffing or are terminated from Ōnin Staffing and 13 weeks elapse, you will lose your insurance.

         You have a “federally qualifying life event”: Qualifying life events are major life events like getting married, having a baby or getting divorced. Call the Advocate line (1-833-751-1096) if you have questions.
